POSITION DESCRIPTION

Collingwood Neighbourhood House (CNH) is a not-for-profit Society serving the Renfrew-Collingwood neighborhood of East Vancouver. CNH’s mandate is to promote the well-being of the Renfrew-Collingwood community by providing leadership and working collaboratively with individuals, families, agencies and other groups to develop and support inclusive, innovative, sustainable initiatives and services that respond to the community’s social, educational, economic, health, cultural and recreational needs.

RISE Community Health Centre (CHC) is the newest department of Collingwood Neighbourhood House. RISE is a community-based CHC that is part of the Primary Care Network strategy and funded by the Ministry of Health. RISE supports the health and well-being of Renfrew-Collingwood residents who are facing barriers to achieving health by offering community-based primary care and health promotion services. Populations of focus include Newcomers, LGBTQ2S+, Indigenous, Isolated Seniors, Youth, people experiencing Homelessness, those working in the Sex Trade, and those experiencing Mental Health and Substance Use issues. Services are delivered by a diverse interdisciplinary team, working in partnership with other Collingwood Neighbourhood House departments and partnering agencies.

Job Overview

  • We are seeking a Licensed Practical Nurse to join our Community Health Centre Team at CNH. The position provides primary care, health promotion and outreach nursing services as part of an interdisciplinary team.
  • Practice within the context of a client and family centered care model and in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice for Licensed Practical Nurses and within RISE polices, guidelines and clinical practice documents.
  • Work within an interdisciplinary team-based care model of Physicians, Nurse Practitioners, Registered Nurses, Social Workers, Dieticians, Physio Therapists, Counsellors and Community Health Workers to assess, plan implement and evaluate and document client care and collaborate with other Community Care Services.
  • There will be opportunities in this role for case management; wound management; administration of medications and immunizations; performing phlebotomy; preforming mental health and addiction care; and chronic and infectious disease management.

 

Skills and Qualifications

  • Graduation from a recognized program for Practical Nurses.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two (2) years recent experience working as a Licensed Practical Nurse providing care through holistic mental health, substance use and primary care lens or an equivalent combination of training, education and experience.
  • Knowledge of Community Health Nurses Standards of Practice and Guidelines for clinical practitioners
  • Current CPR Certification
  • An egalitarian philosophy and non-judgmental attitude (i.e. gender, culture, race, sexual orientation, language, ability and more)
  • Previous experience providing culturally safe, trauma-informed care to marginalized populations
  • Training and demonstrated ability to provide nursing assessments and interventions related to common diagnosis and specific clinical situations relevant to vulnerable populations (e.g. chronic disease, infectious disease, chronic pain, addictions, mental health)
  • Demonstrated ability to perform focused secondary assessments, integrate information from a variety of sources including diagnostics, and develop plan of care relevant to the priority populations
  • Demonstrated ability to employ teaching strategies to transfer knowledge to engage clients, families and groups in planning and self-management of care
  • Experience and comfort drawing blood
  • Completion of BCCDC Online Immunization Competency Course for Licensed Practical Nurses or
    willingness to achieve the same shortly after starting position
  • Demonstrated ability to provide treatment planning, counseling, crisis intervention, and case coordination
  • Demonstrated ability to communicate effectively, both orally and in writing, one-on-one and in group settings, with colleagues, clients and their families
  • Demonstrated ability to provide health teaching in a group setting
  • Demonstrated ability to effectively employ conflict resolution and reconciliation techniques and approaches
  • Experience promoting collaboration, capacity building, teamwork, and communication within diverse interdisciplinary teams
  • Experience applying strategies or practices to address social determinants of health and health inequity
  • Broad knowledge of crisis intervention and counseling skills
  • Broad knowledge of mental health issues and substance use and treatment, including harm reduction approaches
  • Broad knowledge of local community health and social service resources and referral options
  • Excellent planning, prioritization and time management skills
  • Ability to utilize Electronic Medical Records, word processing, Internet and email software
  • Ability to provide services through outreach
  • Physical ability to perform the duties of the position
