CACHC Membership

Annual membership dues in CACHC are tied to your health centre’s Annual Operating Budget, defined as your total operating budget from all revenue streams. Specific dues amounts are listed below in the membership form. The annual membership period runs from April 1 to March 31. Become a member or renew your membership via our secure, online payment form. If you prefer to pay by cheque, please complete this hardcopy Membership Form and submit to us along with your cheque.
