Knowledge Translation & Exchange
CACHC strengthens the capacity of CHCs across Canada by transforming research and expertise into practical, actionable knowledge. Through national knowledge exchange initiatives, research collaborations, and resource development, we help CHCs share best practices, highlight the impact of their work, and apply evidence-based insights to advance health equity. By fostering collaboration and making critical information accessible, we support CHCs with the tools they need to effectively navigate operational, governance, and policy challenges. Together, we are building a stronger, more connected CHC sector to drive meaningful change in community health.
The Community Vaccination Promotion-National (CVP-National) Project
PROJECT OVERVIEW
The CVP-National Project, led by the Canadian Association of Community Health Centres (CACHC), is dedicated to improving COVID-19 vaccine uptake among equity-deserving populations across Canada. Originally funded by the Public Health Agency of Canada (PHAC) from 2021 to 2023, the project worked in partnership with Community Health Centres (CHCs) and provincial CHC associations to build vaccine confidence, address misinformation, and overcome access barriers among equity-deserving populations across Canada. During its initial phases, the project supported localized vaccine promotion efforts and shared best practices across the country.
In 2024, the project received an extension to deepen its impact and further support equitable vaccine access. This new phase focuses on curating, adapting, and disseminating culturally and linguistically accessible vaccine resources, tools, and information tailored to diverse communities. By leveraging CACHC’s network of 100+ CHCs, the project empowers healthcare providers to implement evidence-based, equity-informed approaches that address the specific needs of equity-deserving communities. These efforts include tackling vaccine hesitancy, combating misinformation, and promoting trust in vaccines.
Through its extensive platforms—including Communities of Practice and the Knowledge Hub—the CVP-National Project facilitates ongoing collaboration, resource sharing, and knowledge exchange for CHCs and healthcare providers across the country. The project also emphasizes increasing health literacy and improving access to vaccines, ensuring that strategies are tailored to the unique challenges faced by vulnerable communities.
The CVP-National Project is creating lasting, positive changes in Canada’s healthcare landscape by increasing health literacy and building vaccine trust, particularly for equity-deserving populations, while expanding its reach and deepening support to drive improvements in vaccine uptake, health equity, and health outcomes for all Canadians.
