Community Health Centres (CHCs) play a vital role in the healthcare systems across Canada. CHCs navigate the diverse and complex systems that integrate a range of team-based primary care with health promotion programs, preventative care, community health initiatives and social services to address the social determinants of health to ensure equitable access for individuals, families, and communities across the country. This comprehensive and holistic wraparound model improves the overall health and well-being of diverse populations.
With support from TD Bank Group (TD) through the TD Ready Commitment, CHCs are increasing efforts to improve health and social systems navigation access to strengthen Canada’s healthcare landscape and eliminate health disparities while improving population health outcomes across Canada.
SYSTEMS NAVIGATION ACCESS & PARTNERSHIP (SNAP) PROJECT

With support from TD Bank Group (TD) through the TD Ready Commitment, the Canadian Association of Community Health Centres (CACHC) launched the Systems Navigation Access and Partnership (SNAP) Project. This funding supports initiatives aimed at advancing one or more of the following objectives:
- To help CHCs adopt, adapt, scale up, and improve systems navigation efforts to improve client outcomes and grow the CHC knowledge and practice base across Canada.
- To help improve overall healthcare access for equity-deserving populations in Canada and overcome barriers to accessing services and continuity of care.
SNAP Project Goals
- Enhance accessibility: Develop strategies/approaches that reduce access barriers, particularly for marginalized populations, thus improving healthcare service accessibility.
- Simplify and improve navigation: Employ staff, training, language, or technological solutions to ensure clients can access the care and support they require.
- Empower clients and communities: Provide staff and clients with tools, information, and support to make informed healthcare decisions.
- Enhance systems navigation: Elevate systems navigation practices to seamlessly connect clients with essential health and social services.
- Improve health outcomes: Enhance health outcomes among marginalized populations in Canada.
