0.5 FTE High Risk Foot Care/Wound Care Nurse-1 Year contract part-time.  Hourly: $26.24-$31.90

 

Belleville & Quinte West Community Health Centre (BQWCHC) is a community-based primary health care organization that provides comprehensive primary health care services, community-focused health promotion and development. The High-Risk Foot Care/Wound Care Nurse is a clinician contributing to the reduction of vascular and structural complications and amputations by using a holistic approach that includes prevention, education, assessment, treatment and acute intervention of the foot and lower leg.

KEY AREAS OF RESPONSIBILITY

  • Assesses and provides therapeutic treatments to clients with complex foot care issues; at risk for vascular/diabetic complications and lower limb amputations
  • Provides education, support, and counselling for high-risk foot/wound care clients, accessing internal (BQWCHC) and external supportive services and programs, where necessary
  • Provides clinical advice and support to other practitioners, relating to high-risk foot care/wound care
  • Works in accordance to medical directives, as required
  • Maintains inventory of supplies and tools necessary for High-risk foot care/wound care services
  • Participates in the collection and reporting of data for the High-Risk Foot/Wound Care program
  • Acts as Medical Devices Reprocessing Lead for the organization; and utilizing best practices, coordinates reprocessing procedures ensuring appropriate equipment maintenance and supplies/inventory management of equipment and consumables
  • Participates in the development, implementation, and modification of High-Risk Foot/Wound Care and Reprocessing policies, procedures, and medical directives
  • Works collaboratively as a member of the interdisciplinary team
  • Works in a manner that is mindful of the whole person/community within a social context and impacted by social determinants of health

QUALIFICATIONS    

  • Diploma in Practical Nursing from a recognized, post-secondary college or institution
  • Member in good standing with current registration with the College of Nurses of Ontario
  • Demonstrated evidence of advanced foot care and wound care experience, preferably in a community setting
  • Experience in conservative debridement, arterial brachial index assessment, current wound dressings and off-loading devices and techniques preferred
  • Demonstrated completion of IPAC core competencies with Medical Devices Reprocessing through Public Health Ontario
  • Experienced with community outreach, patient education techniques, including work with people facing systemic barriers to accessing care
  • Experience working independently and autonomously, referring to other services/specialists when necessary
