Temporary Full-Time Position: 35 hours per week

Contract Period: March 2025 – November 2025

Salary Range: $36.36 to $42.90 per hour

Location: All LAMP locations with Primary Assignment at East Mississauga Site

The West Toronto Diabetes Education Program (WTDEP) is a program that educates and supports those at risk, with prediabetes or type 2 diabetes over the age of 18 years. Our program spans 5 community health centers in the West Toronto and Mississauga regions.

Job Summary

In this rewarding role, you'll provide essential support and education to individuals at risk or living with prediabetes or type 2 diabetes, helping them manage their condition and improve their quality of life. Collaborate with community healthcare professionals to create tailored care plans, lead educational sessions, individual counselling, outreach and promote healthy lifestyle choices. The incumbent will ensure that all organizational policies are followed and adhere to the overall objectives of the program and College standards are maintained.

Primary Responsibilities

  • Provide care within professional scope of practise for clients at risk, living with pre-diabetes or Type 2 diabetes, taking into consideration client’s level of knowledge, readiness to change, lifestyle habits, and socio-demographic background.
  • Develop health maintenance care plan with client and health care team by integrating information about client’s health status, clinical practice guidelines, and available community resources.
  • Be able to identify and prioritize complex health and social issues.
  • Ensure complete, accurate, and timely documentation of program activities and medical records.
  • Monitor for accuracy, quality, and thoroughness through participation in chart reviews.
  • Communicate information clearly and concisely to clients and health care providers on a regular basis.
  • Experience with case management and case coordination with members of the interdisciplinary team and services within the community.
  • Create and facilitate health education classes and hands-on sessions on various topics related to diabetes (cooking workshops or physical activity classes).
  • Identify and build relationships with various community partners.
  • Present clinical practice guideline updates to health care providers and clinical partners.
  • Provide learning opportunities to students, new hires, and colleagues.
  • Recommend improvements to existing procedures/services.
  • Facilitate teamwork and promote group effectiveness.
  • Respect people of diverse backgrounds and perspectives.
  • Expected to participate in LAMP and West Toronto DEP committee work aimed to improve programs and client care.
  • Other duties as assigned by Manager

Required Qualifications

  • Bachelor’s degree in Human Nutrition, Kinesiology & Health Science or other health related discipline.
  • Must be a Regulated Health Professional in Ontario (Registered Dietitian, Registered Kinesiologist etc.)
  • Current membership in good standing from respective professional college.
  • Hold professional liability insurance in accordance with the requirements set out by the respective college in Ontario.
  • Experience working within a community setting.
  • Three to five years’ experience; preferably in community-based diabetes care.
  • Certified Diabetes Educator (CDE) or willing to attain within the first year of employment.
