POSITION DESCRIPTION | Full Time Contract until March 2027 ($103,485 – 125,780 / year based on 35 hours per week) The Bridge Integrated Care Hub ("The Bridge") is a collaborative, low-barrier program that provides essential support, social services, mental health, addictions and primary health care to individuals experiencing homelessness in Belleville. Operated in partnership with multiple community organizations, The Bridge ensures access to services 24/7, 365 days a year. Through a "one door, one roof" approach, The Bridge fosters recovery, stability, and system navigation for those in need. The Bridge Integrated Care Hub is one component of the HART Hub, which is a continuum of services that also includes Rapid Response and Outreach Teams and Transitional Housing, that collectively support those experiencing mental health, addictions and homelessness within Belleville and Hastings Prince Edward Counties. The Bridge is operated under the auspices of a Consortium of local agencies including Belleville & Quinte West Community Health Centre (BQWCHC), Canadian Mental Health Association HPE (CMHA HPE), Hastings and Prince Edward Public Health (HPEPH), John Howard Society (JHS), Grace Inn Shelter (GIS), and the Enrichment Centre for Mental Health (ECMH). Belleville and Quinte West Community Health Centre is employer of record for The Bridge Nurse Practitioner. The Bridge NP works as part of a multi-agency, multi-disciplinary team to support people who are experiencing homelessness and/or problematic substance use in the Belleville area. This role includes in-clinic, outreach clinic, and virtual care activity. Focus is on providing episodic and ongoing primary health care to people who are experiencing homeless/at-risk of homelessness and unattached to primary health care. The NP will work collaboratively with an Outreach RN, community outreach team (paramedic & addictions workers), guest services workers, and others both at The Bridge and at BQWCHC. This role has dual accountability to The Bridge Director and BQWCHC Belleville Clinical Manager. Hours are typically Monday- Friday (8:30-4:30), but allow some flexibility for evenings, based on client needs. KEY AREAS OF RESPONSIBILITY
