POSITION DESCRIPTION | Diabetes Educator Registered Dietitian (RD) or Registered Nurse (RN) 1 FTE Permanent Position Longlac Site One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services. We are currently seeking a contract 1 FTE Registered Dietitian (RD) or Registered Nurse (RN who as a valuable member of the Community Health Centre team will provide individual and group diabetes education programs and work collaboratively within an interprofessional model of care team. The position has a competitive salary, the Health Care of Ontario Pension Plan and an excellent group health benefit package. QUALIFICATIONS:
Eligible for the Community Commitment Program for Nurses (CCPN) that provides $25,000 in grant funding to eligible nurses in exchange for a 2 year commitment. Click here for eligibility criteria.
