Preventative and Health Promotion Primary Care Coordinator
1 FTE
Thunder Bay Site

One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.

We are currently seeking a 1 FTE Coordinator who as valuable member of the Community Health Centre team will provide program coordination of the preventative and health promotion primary care program to unattached children at the Thunder Bay site. The Coordinator of the inter-professional care team focus will be on providing primary health care services, disease prevention, health promotion, health education and building strong community partnerships. This is a new funded position.

The position has a competitive salary, the Health Care of Ontario Pension Plan and an excellent group health benefits package.

QUALIFICATIONS:

  • Post-secondary education in health, public health, business, or social service related field.
  • 2-3 years supervisory experience developing effective interprofessional teams.
  • Knowledge and experience developing strong community and social service partnerships to promote preventative and health promotion primary care services to unattached children and their families.
  • Demonstrated values alignment with organizational vision, mission and values.
  • Awareness of and sensitivity to health issues particularly within individuals who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQI+, newcomers, non-insured).
  • Exceptional written, oral and presentation skills.
  • Exceptional leadership, supervisory, organizational and time-management skills.
  • Able to work independently and in collaboration with others.
  • Excellent problem solving and critical thinking skills.
  • Demonstrated commitment to the principles of equity, inclusivity, respect and cultural competence is required.
  • A current driver’s license and access to a vehicle.
