SALARY RANGE: $34,195 - $35,201 (FULL-TIME EQUIVALENT)

South Riverdale Community Health Centre is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. The Toronto Community Hep C Program (TCHCP) is a community-based, collaborative model of Hepatitis C (HCV) treatment, support and care delivery focused on improving access to treatment for people who use drugs and/or alcohol. The HCV Community Support Worker will work within the TCHCP providing support to program members as well as supporting and assisting with the program activities. The HCV Community Support Worker will work together with the rest of the team to improve existing services and projects and in the development of new initiatives. The HCV Community Support Worker will work as a part of a multidisciplinary health and social service team.
This position is expected to work onsite.
Responsibilities:
• Distribution of harm reduction materials and harm reduction education
• Community outreach and HCV education
• Co-facilitation of groups, supervision of drop ins
• Public speaking, public education and advocacy
• One on one support to program members, including appointment accompaniment
• Referrals to appropriate resources
• Assist with preparation for groups and with special projects/community events
• Support members through testing results, living with HCV and/or assessing and sustaining treatment for HCV
• Assist with research/surveys/program evaluation
• Record keeping, maintaining accurate records and documentation of services
• Attend meetings and trainings
• Attend regular supervisory/support meetings
• Maintain confidentiality and appropriate boundaries
• Act as a role model for program clients
• All other duties as required
Qualifications:
• Graduate from Peer training program or equivalent
o Engagement in the TCHCP or other peer training programs may be considered due to the suspension of Peer Training during the COVID-19 Pandemic
• Lived Experience of HCV and/or HIV, as well as substance use, homelessness or mental health issues
• Demonstrated respect for diversity, including sexual orientation, gender, race, ability drug use and sex work
• Demonstrated sensitivity to others
• Demonstrated respect for diversity, equity, anti-racism and accessibility with the ability to work collaboratively and positively with others
• Ability to function as a member of a team
• Good communication and listening skills
• Open to feedback
• Good organizational, coordination and interpersonal skills
• Confident in establishing connections and maintaining strong boundaries
• Basic knowledge of email, internet and computer use
• Excellent Knowledge and practice of Harm Reduction including substance use resources and supports
• Knowledge of HCV/HIV
• Ability to prevent and de-escalate conflicts
• Experience in navigating systems and advocating for illicit drug users rights / needs (including the criminal justice system, medical and hospital systems, mental health systems, traditional drug treatment systems, etc.)
• Ability to be punctual and work consistent hours
• Police clearance documentation will be required for employment
• Proof of double COVID-19 vaccination status will be required for employment
