POSITION DESCRIPTION | Nurse Practitioner – Health Equity Program
Belleville and Quinte West Community Health Centre (BQWCHC)
Full Time Contract until March 2027 ($103,485 – 125,780 / year based on 35 hours per week)
BQWCHC is seeking a Nurse Practitioner to work as part of an integrated team with BQWCHC clients and community members from priority, equity-deserving populations (e.g. Newcomers, Migrant Workers, 2SLGBTQIA+, Seniors, those living in poverty) in the Belleville and Quinte West area. This role includes in-clinic, outreach clinic, and ‘home’-visiting activity. Focus is on providing episodic and ongoing comprehensive primary health care to clients/community members with unmet health and social needs, including those unattached to primary health care to enhance well-being through targeted assessment, treatment, and prevention strategies aimed at reducing health inequities. The NP will work collaboratively with an outreach RN, a comprehensive interdisciplinary team within BQWCHC, and others.
KEY AREAS OF RESPONSIBILITY
- Provide client-centered primary care, involving clients as partners in their care, recognizing the social determinants of health and associated health risks, and taking the stance of a health advocate.
- Perform clinical assessments and health information gathering for clients with complex needs in both a rostered practice model and an outreach clinic model.
- Provide nursing services, including health education and clinical services, which address the biopsychosocial needs of clients, including those with multiple comorbid health conditions, mental health diagnoses, and/or concurrent disorders involving substance use, according to the College of Nurses of Ontario and Nurse Practitioners Association of Ontario standards of practice and evidence-based practices.
- Contribute to the organizational process to provide same-day access to clients of the Centre, including being available to see clients on another provider’s roster in specific scheduled appointments, as required.
- Participate in the Centre's on-call services on rotation to provide after-hours service as directed.
- Actively participate, seek, and provide consultation within an inter-professional model of care.
- Contribute to providing more accessible targeted services to community members from equity-deserving populations by participating in case conferences; making referrals and providing system navigation/connection to interdisciplinary team and external resources and supports.
- Provide input/feedback to support the development, implementation, monitoring and evaluation of treatment, education, counselling and health promotion programs and services for clients, especially those from equity-deserving populations.
- Work in a manner that incorporates health promotion, harm reduction, trauma-informed care, strength-based and anti-stigma approaches, recognizing the social determinants of health.
- Document care provided to clients in accordance with CNO standards.
QUALIFICATIONS
- RN (EC) license to practice from College of Nurses of Ontario.
- Experienced as a Nurse Practitioner, with preference to those with experience in primary health care or in a community/outreach setting.
- Familiarity with addictions medicine and best practices as outlined by META:PHI Ontario, preferred.
- Experience working effectively with communities and individuals who face barriers accessing health care and other supports, including those with complex psycho-social issues, mental health, and substance use.
- Familiarity with health and social services in the Quinte area.
- Philosophy, values, and practice consistent with client-centered, strength based, harm reduction and trauma-informed approaches to care.
- Demonstrated problem-solving, organizational, and interpersonal skills with an ability to work effectively and efficiently in an integrated team environment and independently as required.
- Ability to communicate with clients in an easily understandable manner, practicing active listening to understand client needs and coaching clients as collaborative partners in their healthcare.
- Competency in facilitating client group activities.
- Ability and willingness to work in both an office-based and outreach mobile clinic models.
- Ability to work a flexible schedule, including evening hours.
- Fluency in French and/or Spanish an asset
- A valid driver’s license: reliable vehicle and willingness to travel within the Quinte area is required.
