POSITION DESCRIPTION

Full Time Contract until March 2027 ($103,485 – 125,780 / year based on 35 hours per week)

The Bridge Integrated Care Hub ("The Bridge") is a collaborative, low-barrier program that provides essential support, social services, mental health, addictions and primary health care to individuals experiencing homelessness in Belleville. Operated in partnership with multiple community organizations, The Bridge ensures access to services 24/7, 365 days a year. Through a "one door, one roof" approach, The Bridge fosters recovery, stability, and system navigation for those in need. The Bridge Integrated Care Hub is one component of the HART Hub, which is a continuum of services that also includes Rapid Response and Outreach Teams and Transitional Housing, that collectively support those experiencing mental health, addictions and homelessness within Belleville and Hastings Prince Edward Counties.  The Bridge is operated under the auspices of a Consortium of local agencies including Belleville & Quinte West Community Health Centre (BQWCHC), Canadian Mental Health Association HPE (CMHA HPE), Hastings and Prince Edward Public Health (HPEPH), John Howard Society (JHS), Grace Inn Shelter (GIS), and the Enrichment Centre for Mental Health (ECMH). Belleville and Quinte West Community Health Centre is employer of record for The Bridge Nurse Practitioner.

The Bridge NP works as part of a multi-agency, multi-disciplinary team to support people who are experiencing homelessness and/or problematic substance use in the Belleville area.  This role includes in-clinic, outreach clinic, and virtual care activity.  Focus is on providing episodic and ongoing primary health care to people who are experiencing homeless/at-risk of homelessness and unattached to primary health care.  The NP will work collaboratively with an Outreach RN, community outreach team (paramedic & addictions workers), guest services workers, and others both at The Bridge and at BQWCHC. This role has dual accountability to The Bridge Director and BQWCHC Belleville Clinical Manager.

Hours are typically Monday- Friday (8:30-4:30), but allow some flexibility for evenings, based on client needs.

KEY AREAS OF RESPONSIBILITY

  • In collaboration with Outreach RN, and independently, provide access/facilitate access to episodic primary care services with the goal of establishing a trusting and therapeutic relationship and optimizing connection with physical, mental and social health supports; ongoing assessment of clients for readiness to transition to full-time primary care providers, as appropriate
  • Perform assessments, diagnoses, treatments, health education/counselling for issues relating to substance use, sexual health, infectious disease, etc., according to the College of Nurses of Ontario and Nurse Practitioners Association of Ontario standards of practice and evidence-based practices
  • Assess for, prescribe and monitor appropriate therapeutics for substance use disorder (including polysubstance use disorder)
  • Work from a harm reduction perspective
  • Contribute to the development, implementation, monitoring and evaluation of medical protocols and directives, as warranted
  • Function as a subject matter expert and resource to primary care team members and agency staff in the area; raising awareness and competency with treatments/supports for substance use disorder and other issues faced by people experiencing homelessness
  • Document care provided to clients in accordance with CNO standards in BQWCHC electronic medical record

QUALIFICATIONS

  • RN (EC) license to practice from College of Nurses of Ontario
  • Experienced as a Nurse Practitioner, with preference to those with experience in primary health care or in a community/outreach setting
  • Training, certification from a recognized program in safe prescribing practices for addictive medications and management of substance use disorders in primary care
  • Familiarity with addictions medicine and best practices as outlined by Meta:Phi Ontario, preferred
  • Experience working effectively with communities and individuals who face barriers accessing health care and other supports, including those with problematic substance use, mental health, and complex psycho-social issues
  • Philosophy, values and practice consistent with client-centred, harm reduction and trauma-informed approaches to care
  • Demonstrated problem-solving, organizational and interpersonal skills with an ability to work effectively and efficiently in a team environment and independently as required
  • Flexibility and a willingness to work constructively in a complex team structure.
  • Ability and willingness to work in an unstructured clinic/street-based model, while maintaining vigilance for the importance of the strategic goals of the program
  • A valid driver’s license; reliable vehicle and willingness to travel within the Quinte area is required
