POSITION DESCRIPTION

Responsibilities

  • Provide a range of primary care services to the patients of the Centre de santé Univi Health Centre (CSUHC); perform client assessment, follow-ups, triage, screening, and counselling.
  • Collaborate with nurse practitioners, physicians and other healthcare professionals to develop and implement individualized care plans.
  • Participate in the development, delivery and evaluation of educational and health promotion programs.
  • Maintains accurate and timely records and encounter data in patient electronic medical record (EMR).
  • Participates in chart and case review.
  • Communicates information clearly and concisely to patients and health care providers on a regular basis.
  • Educate patients and their families about healthcare, treatments, and procedures.
  • Participate in the assessment and reassessment of patient care needs.
  • Adhere to policies and procedures as well as professional standards.
  • Other duties as assigned by the Program Director.

Qualifications

  • Bachelor’s Degree in Nursing, or Diploma with relevant experience.
  • Membership in good standing with the College of Nurses of Ontario.
  • Work experience in a primary care setting preferred.
  • Knowledge and ability to assess the needs of various age groups.
  • Experience in the planning, implementation and evaluation of various health-related topics in a community setting.
  • Superior problem-solving skills with the ability to prioritize and time manage in a demanding office environment.
  • Knowledge of community resources, especially in regards to health and social services.
  • Ability to work within an interdisciplinary team.
  • Current certification in Basic Cardiac Life Support (BCLS).
  • Bilingualism preferred (English and French).
  • Solid working knowledge and ability to navigate a PC windows environment, including shared drives and Microsoft applications.
  • Valid Ontario’s driver’s license and access to a vehicle, would be an asset.
