| POSITION DESCRIPTION | Anticipated contract end date February 2026 Working to full scope supported by multiple medical directives, the Registered Nurse is part of an interdisciplinary care team of Physicians and Nurse Practitioners and external community partners. The clinical team is predominantly an outreach mobile team that works in several highly populated areas of the city with individuals that are underhoused or marginally housed who have experienced barriers to traditional medical and social supports. This team is supported by Social Workers, Outreach Workers, Community Developers and support staff specializing in the areas of Mental Health, Poverty & Homelessness, and Diabetes & Chronic Diseases while out in the community.
