Outreach Registered Nurse

Belleville and Quinte West Community Health Centre (BQWCHC)

Full Time Contract until March 2025 (with possibility of extension) - $35.80 – 43.52 / hour

BQWCHC is seeking a Registered Nurse as part of a team working with people in the Belleville area who are experiencing homelessness and/or use substances. This role includes in-clinic, outreach clinic, and ‘home’-visiting activity. The focus of the position is to provide episodic and ongoing primary health care interventions to people who are experiencing homeless/at-risk of homelessness and who are unattached to primary health care. The RN will collaborate with an outreach NP but will also work independently, utilizing medical directives where required. Additional supports include a comprehensive interdisciplinary team within BQWCHC; peer supports and drop-in staff at ‘The Bridge’ – an evolving drop-in service in Belleville seeking to provide wrap-around services for those experiencing homelessness or at risk of homelessness.

 

This program is offered to March 31, 2025, and is a partnership between Bridge Street United Church (BSUC), Belleville & Quinte West Community Health Centre (BQWCHC), Hastings and Prince Edward Public Health (HPEPH), John Howard Society (JHS), Grace Inn Shelter (GIS), Canadian Mental Health Association HPE (CMHA HPE), and the Enrichment Centre for Mental Health (ECMH).

Hours of operation are typically Monday- Friday (8:30-4:30), but allow some flexibility for evenings and weekends, based on client needs.

 

KEY AREAS OF RESPONSIBILITY

  • In collaboration with Outreach Nurse Practitioner and independently, provide access/facilitate access to episodic primary care services with the goal of establishing a trusting and therapeutic relationship and optimizing connection with physical, mental, and social health supports; ongoing assessment of clients for readiness to transition to full-time primary care providers, as appropriate.
  • Provide health education and clinical interventions (e.g., wound care and immunizations), and for issues relating to substance use, sexual health, infectious disease, etc., according to the College of Nurses of Ontario standards of practice and other evidence-based guidelines.
  • Promote harm reduction including distribution of related safer use supplies/naloxone, education, and training.
  • Contribute to the development, implementation, monitoring and evaluation of medical protocols and directives, as required.
  • Work in a manner that incorporates health promotion, harm reduction, trauma-informed care, cultural sensitivity, and anti-stigma approaches, recognizing the social determinants of health.
  • Document care provided in accordance with CNO standards utilizing electronic medical record.

 

QUALIFICATIONS

  • Valid RN license to practice (College of Nurses of Ontario)- no restrictions; experienced in a primary care/community health setting preferred.
  • Experience or passionate interest in outreach nursing; new graduates welcome so long as interest and philosophy is congruent with supporting those facing systemic barriers to accessing equitable care.
  • Ability to work autonomously; establish and maintain appropriate professional boundaries; ability to support clients from a strengths-based approach; supporting goal-setting that aligns with client capabilities and circumstances.
  • Experience in provision of wound care as an asset; or willingness to obtain training upon hiring.
  • Ability to work in an unstructured clinic/street-based model; intrinsic motivation/accountability.
  • A valid driver’s license: reliable vehicle and willingness to travel within the Quinte area is required.
  • Up-to-date CPR certification
